My RSVP card is flat - it doesn't fold over. Do I put it into the RSVP envelope or leave it out and put the flap over it? And it goes in size order with the invite on the bottom and the rest on top of the invite? Thanks!

I think it looks better to leave it out and put the flap of the response envelope over it, that way it's out already for people to fill in, then they can put it into the envelope themselves. Everything in the invite should go in size order from the bottom (largest) to top (smallest). HTH!!!
