I had posted last week about one of my assignments (field trip based on my thematic unit for science) well now I need to create the entire trip. Do any of you have permission slip guides that you can send to me? I have no clue what's supposed to go on them! Do I ask for chaperones on this form too? Also how do you go about getting permission from administration? This is all part of the assignment!

is it just the permission slip part you need help with or coming up with the whole idea of a trip that you could take them on that ties into a Science subject?

I have ideas as to different places to go, but my professor is asking us to create a permission slip to give to parents (if I remember correctly any costs related to the trip goes on this form and asking for volunteer chaperones). I also need to know how I would go about asking administration to take the trip. I imagine I would need some kind of formal letter with learning objectives, how many chaperones would be needed, the cost of the trip, where the money is coming from, transportation, etc. Am I missing anything else?

You know what is funny about this....when you get a job teaching....the school or district has a form letter. All you have to do is fill in the blanks.
Here's what you need to include:
Date
Complete address of the field trip
time of departure from school
when the studens will beback at school
how will they get to the destination (bus, walking, etc)
cost
When payment is due
Just type it out as a letter to the parents and underline the important information to make it stand out.
make a tear off section at the bottom
Child's name
will/will not attend
Emergency contact info
allergies/ meds
Check here if you would like to chaperone. The first 2 parents to respeond will be selected. (Or it can be a random drawing)
