actually most maitre'd split the tip with the banquet manager so dont tip him separately since he will get a tip from you regardless. You can ask your RH first. A bride posted a thread today infact about how much goes to the BM. She was a maitre'd and said from the amount you tip the maitre'd she had to give 30% to bm then 20% to staff so many maitre'ds only get 50% of what you tip them. Generally they say to tip a maitre'd about $2-5 per person. I guess the day of you can decide what they deserve by their performance

also don't forget about hair and make up. If you are having a videographer then tip for them
