tipping list

can anyone share with me your tipping list and how much you gave or expect to give? so far i have:

Florist Delivery People
Hair
Makeup
Limo Drivers (maybe - i have to see if grat is included)
2 Altar Servers (stated on church fee sheet)
Sexton (stated on church fee sheet)
Bagpiper for church
Steel Drum Guy for CH
Photographer
2nd Photographer
Photog Asst
Videographer
DJ
MC
Maitre D
BA

...am i missing anyone?

and also for those of you that used vendors that owned their own business, how much did you tip them?

Florist Delivery People - $0 (he was the owner & wouldn't take the tip)

Hair - $0 (she is a family friend & we went to her house that morning, but she recently had a baby so we made up a gift bag of some cute clothes)

Makeup - $30 (she is also a family friend, she came to my parents house that morning & did an outstanding job)

Limo Drivers - $0 (it was included) the company threw in an extra car for after the reception at no charge but we had to tip the driver $50.

2 Altar Servers - $10 each

Photographer/Videographer - $25 each (we had 4 people in total) & we also served them dinner (they also owned their own company)

Band - $37.50 each. (initally we put $25 each in the envelope but DH was so happy that he gave them extra)

Maitre D & BA - they where both included in our contract so I do not remember
