How are you guys deciding on tipping your maitre d', waitstaff and bartenders?

I was told everyone was included except the bridal attendant and maitre d'. We were told bridal attendant gets between 50-100 and maitre d' gets between 2-4 per person.
I heard most of my guests tipped the waiters b/c they were amazing- drinks never got half empty.

Everyone was included except the maitre'd and the bathroom attendants so those are the only ones we tipped above what we paid the RH.
