Tipping - we're clueless

Ok, here's our list of people to tip. We have no idea how much to tip. Can you share what you did/are doing?

Limo driver
Flower delivery
Videographer (owns business)
Photographer (owns business)
DJ (one guy acting as both dj and emcee)
Bridal Attendant
Maitre'd
Hair (coming to me)
Makeup (coming to me)

Limo driver $50 per driver
Flower delivery $25 and I still tipped the Florist another $100
Videographer $100.00
Photographer $150.00 and $75 for his assistant (both my photog & video owned their own company as well)
DJ (one guy acting as both dj and emcee): We didn't tip our band. We paid over $9K for them and felt it was more than enough
Bridal Attendant: Didn't have one
Maitre'd: price was built in the cost of the RH, but rule of thumb is $3-5 per person
Hair (coming to me) : $100.00
Makeup (coming to me) (same as my hair lady and she came to me as well, she also owned her owncompany)

I didn't even think to tip the photographer.

In my opinion, you do not need to give hair and makeup any more than 20% of the service. Some people even give 15%. They charge more when they have to travel to you, it's not needed to give a higher tip for that. If the makeup costs $150, the tip should be $30-$35. If it was only $50 the tip should be $10-$15.

I also read online that the photographer should be tipped between $50-$100.
