They didn't let my sister do that, but when she went someone was filling out the application for her.

I've read on here that some people were able to, but I think they filled out the application themselves and wrote it in.

Re: two last names

We just filled out our marriage application yesterday. These were the options on the back as far as name changes go:

Option 1: the surname of either spouse; or
Option 2: any former surname of either spouse; or
Option 3: a name combining into a single surname, all or a segment of, the premarriage surname, or any former surname, of each spouse; or
Option 4: a combination surname separated by a hyphen, provided that each part of such combination surname is the premarriage surname, or any former surname, of each of the spouses.

I got this off this website:

city clerk

ETA: we did this in the Kew Gardens office and we filled out the application ourselves. I hyphenated, so I'm not sure what would've happened if I just wrote in both names without the hyphen.

from what i understood, is that you can legally change your name to his surname, but you can still use maiden married....as long as you're not using it to 'defraud'. that's what i'm probably going to do.....have his last name on the wedding certificate, but sign stuff maiden married.

I have ALMOST the same problem... I love my last name and FH's last name is a 15 letter Greek name! I think I'm only legally changing my name if we have kids. Otherwise I don't think I can handle being 'Rebecca Txxxxxxx Hxxxxxxxxxxxxxx!'

That's what I'm doing too.

Re: two last names

I did what someone said their sister did -- when I filled out the application myself at the clerk's office, I just filled in the 'new surname' as MAIDEN LAST with a space in between.

The marriage license is printed that way.

I will likely just use FH's last name, but I want my SS card to have both. My passport and driver's license will have both.

I liken it to having a two word last name, like Van Dyke or de Leon.

I'll let you know what happens in a month or so when I'm changing my SS card etc.
