Vacuum Survey

I was wondering if anyone would mind taking this vacuum survey. This is part of my research project for class.

It's a survey on vacuum utilization and purchasing and since most of us are registering for a vacuum it would be great to get your feedback. The survey should take no more than 5 minutes to complete.

Thanks in advance for your help!
