We got our marriage certificate...now what??? Update w/more questions
Got it in the mail last night!!!! So surprised at how quickly it came.
I will be changing my name, so what/where to do?
****Oh, and I just found the form, downloaded it, and filled it out. Do I sign my name as it is now, or as my married name?
*****Do I NEED to bring my passport??? I have my ORIGINAL marriage certificate, ORIGINAL SS card, and my driver's license. Is this enough?

To change your name you need to go to the SS office.
Bring the 'original' document with photo ID. You can check their website for all the requirements. I had to bring my SS card also when I went for a name change but some people said they did not need to. You can also print the form online so you are ready when you go to the SS office.
Once you have are done with the SS office, you can go to the DMV, again, bring the marriage certificate. Driving license, car title, etc...
Contact you HR to change your name also (paychecks, 401K, etc...). You may have to wait to get the SS card in the mail. (My work would not do it until I gave them a copy of my new card).
Then, call your credit cards. Some will ask you for the 'documents'. I did not see the need for me and they just changed it over the phone.
Don't forget your bank for all your accounts.
You can have your name changed on your passport.

Everything is covered in the post above...You do not have to wait to go to DMV to get your new SS card though...
Make sure if you have any loans or what not, you change your name with that bank too!
Cell phone bills (you need a copy of your new drivers license & marriage certificate).

If you are a Brooklyn resident - you must go to the Brooklyn SS office. You can't apply for a new card at any other location if you live in Brooklyn.
Oh, and I just found the form, downloaded it, and filled it out. Do I sign my name as it is now, or as my married name?

You have to bring that orginal copy and a photo id to both DMV & SS. You can even save time by going on-line and printing up the forms at home.

You don't need your passport. One form of photo id is enough. I went to the SS office in Manhattan and didn't need my SS card. All I needed was the form and a photo id.
