you apply at your town hall. In Smithtown it was I think $45.

Don't get it too early. I think it's only good for 30 or 60 days... (Can't recall)

yea I know it's good for a certian amount of time. I live in queens and I was going to go to manhattan to get it. But they told me you can get it in manhasset I will try there. thanks!!

thats where i was going to suggest. i just picked up the paper work on it when i applied for a new birth certificate. it also said you can't apply for one more than 30 days in advance i believe.
