I actually just checked on this tonight...

You need:
Birth Cert or Baptismal Cert

AND

Passport or Drivers License

Are u going in LI or NYC? Because NYS (LI) it's different that NYC in terms of requirements. We went to Queens and all we brought was our passports...BUT, LI requires your birth certificate, baptismal, etc.

For Manhasset or Queens its okay, further in LI you must have your birth certificate. We got ours last Friday at Manhasset with no hassle at all. We were in and out in 25 mintues!

what about the town of hempstead? the town clerk's website said u need 2 forms of id and 1 must be photo?
can u use a passport and drivers lisence for that one?
i have no idea where my birth certificate is...

I think you need your birth certificate. We did because (at least in Suffolk County) they put your birth information on the marriage license.

You can get a duplicate birth certificate. I had to do that because I couldn't find mine.

This site http://www.vitalrec.com/ explains how to get a certified copy of your birth certificate and you can even order it through them.

Were going in Staten Island...not sure what they require but being it's one of the 5 boroughs it may be the same as Queens.

My birth certificate is at my mom's house in Brooklyn...I don't feel like driving all the way over there just to get it. I plan on bringing my passport and drivers license to the one in Staten Island.
