My ceremony starts at 8pm...and the photographer wants 4 hours for pics. So I need to be ready by 4pm.
Where I am getting hair and makeup done is about 15 minutes from country club.
What time should I book my hair and makeup for and which come first?

I would probably get hair and makeup done at around 1:30...just to give yourself time for everything, plus get dressed. I would get my hair done first.

Tell your hair stylist and make up artist the time that you need to be finished by and they will be able to tell you when to begin. I guess it depends on how elaborate your hairdo will be.
Hair is done first and then makeup. Although makeup can be started while your hair is setting (moisturizer, foundation etc.)
