The first one.

The second is informal.

Re: when addressing envelope

according to proper etiquette, you're not supposed to include 'and family' on the outer envelope, but I am going to do this because most of my relatives won't look on the inner envelope to see that their kids are invited.

Neither.

Mr. and Mrs. Joe Smith

And then on the inner envelope all of the names of the people in the family that are invited.

Joe, Sally, Jenny, Robby

Re: when addressing envelope

also, you shouldn't use shortcuts and abbreviations on the outer envelope, so write 'and' instead of '&' and write out all states, cities, and street names
(example:
Mr. and Mrs. Charles Brown
123 Snoopy Avenue
Woodstock Heights, New York 12345)

Is there any formal way sof writing the wife's name too, instead of just Mrs. Jane?

Are these wrong?
Mr. John and Mrs. Jane Smith
or
Mr. and Mrs. John and Jane Smith

I know the second one has two 'ands.'
