Personally, I am offended when I see that women's first names are dropped and they only get an 's' (as in Mrs Husband's name)
I know lots of my friends feel the same way and my Fi who is not American thinks that it is so strange that married women lost their first names when they get married.

It may not be traditional etiquette - but I would rather not offend anyone by assuming they go by their husband's name.

Mrs Jane Smith & Mr John Smith is the safest way to go (esp for younger people)

I was reading that for the inner envleope you drop the 'Husband's' first name and just use

Outer Envelope
Mr. and Mrs. [Husband's First Name] [Last Name]

Inner Envelope
Mr. and Mrs. [Last Name]
