where do I get a small grommet tool and grommets?

I want to make fan programs and have them all laid out... However, where do I get the little grommet tool and the grommets that are the right size? A regular one (for like a shower curtain) is just way too big.

I need to make 100 of these things, too. What have I gotten myself into?
