The US Post Office - they have the applications. You will need an original borth certificate and a photo ID, I believe. They will have you get a passport photo (2 I think) and then you can send off the application. Allow 2-3 months to get it.

You can also get it in the town hall, for the town where the hospital you were born in is located.
If you were born in any hospital in the town of Hempstead then you can go to Town Hall on front street and so on.
Going to the town hall allows for you to cut a few weeks of the time and take your picture there.

I just went a week ago. I went to rite aid, took the photo (7.99), I filled out the form went to the post office 97.00 later I was done. It was so quick and easy.
ETA You need 2 forms of ID, 1 photo. Make sure you bring your birth Certificate too. They take it then mail it back to you. They do not accept credit cards as a method of payment so don't forget your check book or cash.

Like the above poster stated. They actually take your original birth certificate and mail it to you weeks later with your passport. SO make sure you don't need your birth certificate for anything when you go.

You can go to Walgreens or CVS for the pictures but the Post Office does the actual application process.
