wording escort (place) cards

Hello-

How formal are you writing out your escort cards.

Are you doing:

Mr. & Mrs. John Smith
Mr. & Mrs. Smith
Joe & Jane Smith

Just trying to get an idea of what others are doing.

TIA!

I know the first way is proper, but I might make mine less formal and go with the third option

I'm doing the names on 2 lines b/c I have a decal in the left corner so it will look like this:

Mr. John Smith
Mrs. Diane Smith

I am going with Joe and Jane Smith. Our wedding isn't super formal to begin with though.
