wording for addressing envelopes

When do you spell out 'and' and when do you use '&'

Mr. and Mrs. John Smith

Mr. John Smith and Guest

Mr. and Mrs. John Smith and family

I am sending out STD's and making the lables and I am stumped.. especially on the last example (you know, including their kids). It just seems like too many 'ands'

ETA: Oh yeah one more example.
My stepgrandfather, his mentally challenged daughter and his girlfriend.....

I thought
Mr. John and Miss Sue Smith and guest

again too many 'ands'

ETA: Do you spell out Road and Street?

Mr. and Mrs. John Smith

Mr. John Smith and Guest

Mr. and Mrs. John Smith and family
--->Formally, for invites, always use and. If you wish on a more informal address such as a STD, an ampersand is fine.

Mr. John and Miss Sue Smith and guest

--->Better, since they are not married is, IMO:

Mr. John Smith and guest
Miss Sue Smith
(becuase he is bringing the guest)

ETA: Do you spell out Road and Street?
--->Again, on a formal invite, yes, on an STD, or something informal, up to you.
Hope this helps.
