wording/set up for reply cards?

I'm printing my own reply cards... how are yours worded and set up spacing-wise??

__ Seat(s) have been reserved in your honor

__ Accept with pleasure
__ Decline with Regret

mine is centered and words like this:

__ Places have been reserved in your honor

Kindly respond on or before January 17, 2007

M _________________________________

__ Accepts with pleasure
__ Declines with Regret

[ __ of __ will be attending]

i contemplated putting that last part on in [ ] , but didn't. however i have seen ppl do this.
