Name Change Questions
I got married a year and a half ago and have yet to change my name
... i have a day off coming up and decided to FINALLY get the process started.I'm filling out the Social Security form online but am a bit confused ... it asks for my parents' SS numbers - do i really need to fill that out or can i just leave those blank?
After the SS Office, can i go to the DMV right after or do i need to wait for my SS card to be sent to me?

I did them same day. I don't know if you have the correct form because mine did not ask for parents SSN. I only needed the form and my marriage license

i didn't do mine the same day but i didnt have the card yet. just bring the marriage certificate/license.

Hmm ... their site said this is the form I need ... maybe they made a change to the form since you did it?

i guess if it's wrong, i'll just get a correct one at the SS Office and fill it out there

That is the correct form, I did not have to put my parents SS#, they said it wasn't necessary. I did fill out the other info about my parents, I think it was birthdate and stuff like that.My SS office told me that I had to wait 3 days to go to the DMV because it would take that long for the SS office to process everything and have it show up online in the records when the DMV looks for it. But it took longer than that for the card to come. As long as the DMV can look up the info on the SS records page, you are fine.

I went to DMV right from the SS office, I didnt have any issue changing my name at DMV. They just want to see the marriage license.Welcome New Vendors
