Florist quote and booking

I am interested in booking a florist for my event on 9/26/09. I live out of state and need to book long distance with a sample viewing at the end of May. I will need:

Brides bouquet
5/6 bridesmaid bouquets
4 corsages
12 boutonieres
12 low centerpieces
cake flowers

Below are some sample pictures of what I am looking for. The wedding colors are sage green and lavender.
