Need quote for car service to JFK

I am looking to quote car service for round trip to JFK from Patchogue

06/30 - need to be at airport 4:30am
7/10 - p/u at airport at 3pm

Also - what is the suggested p/u time on 6/30 - not sure if theres traffic that early and how long it takes to get there.
