UPDATED with Bank Info: Info on Changing Your Name
I know this is a huge concern for all of the brides, so I thought I'd post some info now that I've actually done it. I did Social Security and DMV in the same day.
I went to the Social Security office in Melville and filled out the form there while I waited. I just needed the form, my original marriage certificate (which was waiting for us when we returned from our HM- Thanks Town of Oyster Bay!), and my passport. She gave me a receipt showing I applied for a new social security card. Here's the link that contains all of the info you could want/need, the downloadable Form SS-5 and a link to find your local social security office.
Next was the DMV. Be prepared, apparently you take a new picture. I went to the one in Bethpage. Again, I filled out the form there, gave them my original marriage certificate and old license and I had to pay $10. They gave me an interim license with my new name on the spot. Here's the link for the DMV info, including a link to form MV-44 to change your name and form MV-82 to change the name on your registration and/or title. It says it's $15 to change your name and no fee to change the reg or title, but I only had to pay $10.
UPDATE WITH BANK INFO: I went to Chase in Farmingdale. They asked if I had my marriage license with me, but never looked at it. I just gave my account number and new last name. Kind of scary.

Re: Info on Changing Your Name
Thanks.
So they keep your original marriage license at Social Security?? Did they say why?
I plan on doing DMV and SS the same day too. Probably in the next 2 weeks.

Re: Info on Changing Your Name
this is irrelevant to you guys, but in AZ, they make us wait TWO DAYS after going to Social Security to go to the DMV. what a pain!!

That's weird. You don't even have to go to SS first here in NY. In NY you have to show a SS card to them, so you even if you do go to SS first and then the DMV, unless you wait a week for the new SS card to come, you show them your old one anyway.... this is what I did.
