I will have to change my name and my address when I get married in June. For those of you that have done it already, did you have to fill out two separate forms at DMV?

no, you can do it on one form - i did the same

You can do it on one form.. Go to the DMV website and print out the form, so you can have it already filled out when you go there...

The form I used was the MV 44 and it has a space on it that you can put in your new address.. What form are you looking at?
