we have our own checking accounts where we get our direct deposit and put all our living expenses into our joint account and pay bills from there.

Yes we do
Yes. We combine everything- we have one savings account and one checking. It worked for my parents and his parents. We both believe in the whole when you're married then 'what's your's is mine and what's mine is your's'.
