For those of you who have changed your name on your passport
Did you have to send an original marriage certificate? If so, did you spend the money to get an extra copy, or did you just send your original? Do they send it back with your ammended passport?

I requested extra copies of my marriage certificate, but they sent it back with the amended passport.

NJ is a little different with regard to the marriage certificate- you have to go in person with ID and pay $10 per copy.
So, I purchased 3 copies - for 'display' and 2 for legal purposes.
I send an original copy, with my passport, and the name change form.
There is no charge for changing your name.
You will get your SAME passport back, but on the back cover, they will type 'name changed to Mary Smith on Aug 10, 2005'
They will mail that back with your marriage certificate.
They say it takes 6 weeks, but mine was back in 3 weeks

Jackie--Thank for reminding me. I need to do that too! I will probably just send the original, since everything else we need it for is pretty much taken care of.

It's rolled up inside our certificate holder.

I sent a certified copy since I still need my original to do some more stuff and i didn't want to be w/o it too long.

I sent my one and only original with the passport....I got it back within a few weeks along with the amended passport.

Same here. I made sure I did all the things I needed the original for first before I sent it away. I also made a few copies before I sent it.
