You have to mail it in, with your original marriage certificate. They just amend it so it's sort of anti-climactic when you get it back.
Oh, and I just checked. If you just got the passport within the past year, there's a new form. If it's been longer than that, you now have to pay the same fees as if you were just renewing. Wow, that sux. Anyway, here's the link to the 'Correcting Information' page.

Is there any other way to change it without mailing in my marriage certificate. I never got back my birth certificates from when DH and I mailed them in 3 months ago!

I only know the rules for if you got your passport in the last year - but you do have to mail in the passport along with 2 new photos. Very annoying!
