I typed in 'name change passport' in to google and all the info comes up and you can type your info in and it prints out a form. I need to go get another picture taken because its more than a year since my passport was issued. Very easy to do! Just a pain to get the picture done again!

I just did this recently. At first, I though all I had to do was fill out a form, send in my old passport and send in a certified copy of my marriage license.
I got everything back a couple of weeks later, telling me that I had to get new pictures taken, fill out a whole new form and send everything to another address and pay for a brand new passport (even though my current one had 4 more years left on it).

When my boss changed her name on her passport they added an ammendments page... but since the new ones are coming with chips embedded in them now, I believe they're trying to replace when possible...
...my passport is about the only thing I haven't changed my name on, but that's because I was waiting until after getting hired at a new job to send off my marriage certificate since I don't know how long it will take to process... and I now live in Indiana, so running back to Riverhead to get another copy of my marriage certificate isn't a very easy task if I need it anytime soon again.
