Name Change -DMV

I'm sure this question has been asked a million times, but what do I need to bring to the DMV to change my name? Will my current license and marriage license be sufficient? I'm just changing my name,not my address.

Does anyone know what form I need to fill out?

Also what about SS#, the same thing?

bring everything to be safe... i almost was sent away because they wanted proof of address and my old license was not good enough. i got away with an envelope that had my address and current car insurance info.

for DMV i always bring:
- marriage license
- passport
- birth certificate
- insurance and registration info (make sure to update title and registration if applicable)
- recent utility bill to verify proof of address

for SS# change i brought the above PLUS my existing card. i don't know if they will issue a new one without an old one turned in.

I only showed my current driver's license (at that time ) and my marriage license at the DMV.

At SS, the woman didn't even look at my driver's license. (although she saw I had it) She said that she only wanted to see it if it had my new name on it. She took my marriage license from me. I didn't have my SS card with me because I couldn't find it on that day.

I also had the forms already filled out when I went there.

if you are changing your name on your registration and title, make sure you bring the title. i forgot to do this and they wont change my name until i bring the actual title in. i had no problems changing my name on my license.

I called them and they said to bring a marriage certificate and your social security card with your married name. Its good to bring more though just in case!

i didn't turn in my old license or SS card. wanted to keep it just in case. glad i did because i STILL haven't changed my credit cards. i also wanted to save it because of the (don't laugh) memories.

All you will need is your marriage license, birth certificate and drivers license.
