Need Opinion on Name Change Please

I am going to the DMV tomorrow to change over license, registration and title but I am not sure how I should change it.
I legally changed my name to make my maiden name a second middle name so I technically have two middle names now. On most things I have just been putting my first and last name, no middle initial(s) even if I have used one in the past. Should I put my middle initials on my license/reg and title or no? The only thing that I am worried about is because my new last name is VERY common. Would you just put first name and last name?

Because you said you have a very common name - I would use my middle initials.
I just pulled out my license - I did put my middle initial on there - I don't normally sign with it but on my license its there.

For my license, I'm going to put:

First Maiden Married

(and leave off my middle name...if they let me)
