Passport name change??

I know this has been asked before...but I need to change my name on my passport and I need the passport in my hands by August 5th. What do I do??? Help!! Thanks ladies!!

If your passport was issued less than 1 year ago
you have to mail your current passport, original marriage cert,1 color photo and the name change form DS 5504 and its free

If it was issued over 1 year ago you have mail your passport, orig marriage cert, 1 color photo, Form DS-82 and the fee (you have to pay for this one)

I did mine in January and got it back within 2 weeks.
