URGENT HELP NEEDED regarding how the Passport center amended my passport

I just got mine back today but I think the Passport Center did it wrong! Because there's nothing in it about amending it for my name but on the back page it says it was amended to ADD PAGES TO IT! It says on the last page 'this passport was amended on Sept 21 2004 to add visa pages to passport number xxxxxxxx'

What the heck??????? I filled out the right form for name change. I gave a CERTIFIED copy of our marriage license, and I even expedited it ($60) so I knew I'd get it back in time and now I have to do it again?

So if anyone got a name change - what does it say on the last page of your passport?

What really gets me is that the passport wasn't even used (I renewed it a few years ago thinking I was going to be travelling but then didn't travel after all) so why would they think I would need MORE pages - none were even used up!

Sorry-I misread your post. I think this is an error. Call the regional office -- S. Carolina and they will fix it att no charge.

Then why did they add visa pages and not mention on the back part that the amendment is for name change, not adding visa pages?

I am going to call monday as well.

I got mine back on Friday afternoon and it said this page as 'ammended to insert new name here' and the date , and it was right. If I was you I would be calling them. I cannot believe I had to wait so long for them to just type something on the back-

Question..when you change your name on a passport do they give you a new passport or just make a name change?

Just make a name change. You get the same passport back.

I am too scared NOT to have it expedited again because if I don't expedite it (which is why you pay $60 - the amending itself is free) it takes 20 business days. And in 20 business days I am going to Ireland! What if it take a bit longer - then I'm screwed. So I am going to call them and see if I can bring it to someplace in NYC and have them fix it tomorrow asap for me or just pay and expedite it again. It says on the receipt NO REFUNDS but they screwed it up not me.
