I believe you include the first name on outer envelopes,

Mrs. John Doe (if the husband is deceased)

the husbands name on the outer. this is how it goes if you are using inner envelopes
Outer: Mr. and Mrs. John Doe
Inner: Mr. and Mrs. Doe
or
John and Mary
