I'll get to the point..

How do you address the inner for Mr. and Mrs. Smith who are your aunt and uncle?

Mr. and Mrs. Smith
Uncle Brad and Aunt Angelina

?

I would still use Mr & Mrs

I had fun with some of these and wrote 'Mom and Dad' and stuff like that on some. Very close friends, I put their first names.

I printed all our inner envelopes (and outer envelopes) at home myself so I had a lot of freedom.

If you want them super former, you do 'Mr. & Mrs.' but this is your wedding and you can do whatever you want!

A lot of people don't even notice how the inner envelopes are addressed (or the outer for that matter!).

I think this is a personal preference. I think I will use 'Aunt' and 'Uncle' just because I am most comfortable with this and I think they are most comfortable with this as well. But.. I think Mr. and Mrs. sounds nice too and it might actually be the 'correct' way.
