Any tax people out there?

DH has a side company that he needs to file a return for. We know which IRS form to complete (Form 1065) but not which NYS one (it is a Domestic LLC). He can't find copies of what forms he filed last year (another story for another time). He also can't find his NYS EIN as well.

Does anyone know which NYS form he files for this company he does nothing with?
