Anyone have a sample cancellation letter for a vendor?

We have to cancel a vendor as we have decided to go with someone else. Does anyone has a sample letter, I'm not really sure how to word it. Thanks!!

I don't but I did this recently. I just called them and told them adios... in a nice way of course.

This vendor requires a written cancellation letter sent certified mail! I thought it would be easy, but I just can't word it right!!

I'd just keep it short and simple. Maybe something along these lines:

Dear Vendor:

Please be advised that we have decided to terminate our contract, dated ________, effective immediately. Please acknowledge receipt of this letter by countersigning and returning a copy in the enclosed postage-paid envelope.

Please contact me if you have any questions.

Very truly yours,

Receipt Acknowledged:
