I have a really similar situation. My sister is also young. My cousin and i were born 28 days apart and have grown up together as sisters and bestfriends since we were born. SO, i asked them both to share the honor. But first, i took my sister aside and asked her. Then told her that i was trying to think of a way to make our cousin feel special and that I had once thought of asking her because it would mean so much to her. bla bla bla. My sister was totally okay with it and she was the one who said, what about making us Co-MOHs??!! Am i a good at reverse psychology or what!

anyway, as a way of making Jess (my sister ) feel special, she is going to get to wear a special dress (i.e., different from the others, and it also has a bit of a train) which means a lot to her. plus she told our cousin (jokingly) that she gets to stand closest to me!! it was very cute, but everybody is very copacetic, and i am getting beaucoup support!!

I was a MOH in a wedding w/ a maid and a matron -- nowadays you can do whatever you want!!! But even though they both get the title, only one girl will sign your marriage license. I don't know whether that matters to you or not but other than that there's no problem!

I wanted this too. I only had two girls and wanted to choose them both.
If you are getting married in a RC church check with them first.
My church told me I could have only one. I think it was more about having two girls up on the altar. Anyway, just wanted to let you know about the possibility.
Enjoy your day. And don't worry, your friend will probably still be a great help and support no matter what happens. I hope it works out how you want.
