Carrie - I have the answer for you on the name change
I spoke with my sister, and she said when she went to town hall for the marriage license, she told them what she wanted to do (move her last name to become her middle name). She then brought that to the SS Administration. Then, brought her new SS Card and marriage license to DMV for the license change, etc.
