Do I need originals of my marriage certificate to change my name at Social Security and DMV?

Re: Change of Name
Yeah you bring your original certificate and I think they make a copy. I still have to change mine with my credit cards. After that, you can change your name at the post office, change it with HR at work, phone bill, cable bill, student loan. etc. Then after that you can change your account info on websites. Good luck!

Yes, bring the original and they will make a copy and give it back. I just finished doing this.

You should bring your old SS card (they keep it) and I brought my passport for ID.

this is madness.. but this is what I have to change:
-all utility bills -all bank accounts -IRA's -Insurance (health, car, home, ring) -Registration on cars
and I need to find out if I can change my name on my mortgage without it being a royal PITA.
Good luck. it stinks.

Re: Change of Name
One question..... for Soc Security, did you need to bring your original birth certificate? Is a copy acceptable? I have my original SS card. Do I need to bring the original birth certificate too?
Thanks for the info too!!

Re: Change of Name
I googled 'how to change name after marriage' and came up with this website from Bankrate.com. Looks like they outline everything in order of importance. Hope this helps!

On their instructions, it says copies are not acceptable. So, I brought all originals with me but they never asked for it. I ONLY showed them my marriage certificate. That makes me wonder...

Re: Change of Name
I just did my license a week and a half ago and got it on Saturday. Did my ss card last Tuesday and got it in the mail today. All I needed was my marriage license. No ID or birth certificate was needed. Now I've got to change my credit cards, insurance, etc.

See, that's what I noticed too. The application says original birth certificate as proof of age. My dad used to work for Soc Sec, but he's been retired a while now and doesn't remember what they used to tell people they needed to change their name.
I just printed out the SS and DMV applications. I noticed that there's also another DMV form to change the name on the title and registration, so I printed that too. I just paid off my car in August, so I have to figure out how to get the title too.
Besides credit cards and utilities, I also made a list of all the places where I'm registered, my health insurance and all my doctors, my union and benefits through my job, websites where I'm registered, our apt management company, and any other place I could think of. Since I had to do this when we moved to a new apartment, I had a list already made, so I'm just going over it again and adding anything else that comes to mind.

Wow! you're really on top of things. I think I'll change each account as I come accross them. SS and DMV is the most important right now.

Thanks! Sometimes it's nerve-wracking and feels like too much paperwork, but I have to be on top of things. I have so many follow-up doctors and cancer-related/health insurance related things going on that if I'm not organized, I'd never know what was doing! But even before the cancer, I've always made lists. Helps keep me on track!

I am so sorry. Was the cancer recent? Are you okay now? Both my parents had cancer. I can totally relate.

Re: Change of Name
Ok ladies... I remember reading I think on another post to be sure to white out? i'm guessing on a copy of the license before you go giving all these copies out? Did any of you do this and white out information and if so what? (i haven't received mine back yet so I am not sure what is on it) but for ex. SS #'s etc? I am nervous that there is too much personal information on this form that we are giving mass copies of to so many people.

Re: Change of Name
I changed mine with the DMV, Social Security, IRS, my professional licenses, and credit cards and banks. I needed my original certificate for the DMV, the bank, and Social Security.

Re: Change of Name
I needed the origianl for my bank accounts, DMV & SS. My car lease and other accounts were simply the photo copy.
