Do I need originals of my marriage certificate to change my name at Social Security and DMV?
What else besides credit cards do I have to change?
TIA

One question..... for Soc Security, did you need to bring your original birth certificate? Is a copy acceptable? I have my original SS card. Do I need to bring the original birth certificate too?
Thanks for the info too!!
On their instructions, it says copies are not acceptable. So, I brought all originals with me but they never asked for it. I ONLY showed them my marriage certificate. That makes me wonder...
See, that's what I noticed too. The application says original birth certificate as proof of age. My dad used to work for Soc Sec, but he's been retired a while now and doesn't remember what they used to tell people they needed to change their name.
I just printed out the SS and DMV applications. I noticed that there's also another DMV form to change the name on the title and registration, so I printed that too. I just paid off my car in August, so I have to figure out how to get the title too.
Besides credit cards and utilities, I also made a list of all the places where I'm registered, my health insurance and all my doctors, my union and benefits through my job, websites where I'm registered, our apt management company, and any other place I could think of. Since I had to do this when we moved to a new apartment, I had a list already made, so I'm just going over it again and adding anything else that comes to mind.
Wow! you're really on top of things. I think I'll change each account as I come accross them. SS and DMV is the most important right now.
Thanks! Sometimes it's nerve-wracking and feels like too much paperwork, but I have to be on top of things. I have so many follow-up doctors and cancer-related/health insurance related things going on that if I'm not organized, I'd never know what was doing! But even before the cancer, I've always made lists. Helps keep me on track!
I am so sorry. Was the cancer recent? Are you okay now? Both my parents had cancer. I can totally relate.
No, I actually had cancer as a teenager, and I'm OK now, just that some of my insides are falling apart as I have late effects from the chemo & radiation I still see docs for follow-up, and each organ system has its own specialist. So, lots of health-related paperwork, and I have to change my name in each doctor's office. The privacy act is great, but in terms of paperwork and running around, it's h-e-l-l. I recently changed my address, and I had to physically go to several doctors to change it, couldn't do it over the phone b/c of the privacy thing.

Re: Change of Name
I successfully changed my name with Social Security today. Had to go to their main office in Jamaica (my dad used to work there!). Waited on line for over an hour. They asked for my original marriage certificate, driver's license, and original social security card. They did not ask for my birth certificate.
So, one down, a bunch more places to go!! DMV is next.... ugh.

Congrats! I am going to the DMV tomorrow.
Congrats to you too!!
I may have to get to the DMV sometime next week, or even leave it until the day after Thanksgiving. The 2 possible offices I'd go to are open late only on Thursdays in Garden City and in College Point (Queens). I only found this out late today, and next Thursday is Parent-Teacher conferences both afternoon and evening, so that's out too.
I called my car insurance company to change my name first, because I'm going to have to change my name on my car title and registration too. I was able to get a very nice helpful DMV rep on the phone and she told me to do the insurance first, since the DMV will need my new insurance card to make the changes to my title and registration. This may not be the same for everyone, as I just finally paid off my car in August, so I need to do some paperwork for transferring the title into my name.
