court for name change

Hi all, I keep getting all different responses to this question so I'm gonna try again---

How do I go about changing my name to first middle maiden married?

I am pretty sure this is a legal name change and I have to go to court for it but I don't know how to do that or what to fill out on my marriage license when I go get it.

If you're hypenating you can just do that at the SS office. Otherwise you have to legally change your name at the courts.
