When I booked my date the DJ gave me a packet which had all the info he'd need to know. There also was a breakdown of all the special dances and activities that we could change or omit from our night. I'd call your vendor and see if they have a checklist like that to provide.
My DJ sent me a paper to fill out, which I'll have to bring a month before (he said June 1st which is like a month and almost two weeks early) and finalize our information. I got the paper to fill out about three week ago for a July wedding....they said to work on it over the next couple of months so I'm not crazy at the end.
