Anyone know what form we have to fill out after the wedding to change our name on our drivers license? Does DMV have it on their website?

This should help.....DMV

There is a section on this application that asks you if your name has changed. You have to go to the DMV in person with your marriage lisence and drivers license or other photo ID.

If you own a car, you might want to fill out this form as well to change the name on the title and registration.
