Does anyone know about Disablitly Benefits?

I am not going back to work because of the HBP that i been experiencing.. i thought my Dr was going to cheer when i told him the I was not going.. but now once my FMLA is over who can i contact about Short term Diablility? ANy answers would help

Short term dissability has to be filed through the employer.
It has to be filed I believe like 60 days or something from the last day U worked.
They have a form they send you. Its a DB450 or DB50 form ( i forgot) and u fill a few lines out and your doctor fills something out and then it gets sent back to employer who sends it to your insurance company.
They pay 50% of what u have been grossing for the last 3 months of work.
Checks come weekly till a certain amount of time after baby comes, dont remember how long.
The first person to contact is your benifits department.
