if you want a weekend appointment, i would say maybe 3 weeks in advance, if its a weekday appointment most salons are pretty flexible.

depends on which salon and the time of year.... for me in the fall it took me about a month to get a weekend appt at kleinfleds, but all the other salon were able to accomodate me within the week i called
