girls getting married in RC Church

did your priest give you a form that two witnesses have to sign stating that you aren't married, don't have kids and you're not related?
I need to email it to someone - anyone have it?

I don't have this either, but I recall that the witnesses are signing something at the rehearsal. Is this the same?

i just got it a few weeks ago...we booked the church over a year ago but they called us in last month to talk to us about a few stuff and go over a few things and we were given this sheet with two witnesses having to sign it
