bloomingveils.com I had cathedral veil custom made, pics are in album (I haven't received it yet, but she sent me pics with the lace pinned before she finished it so she could make changes if she needed to - I'm having her make the back a little more round).

Occansey Designs - She will custom make any veil for you (and her gallery has TONS of pics) for a great price
TruPink - Also great prices, but basic customization. I got my blusher from here.
Veil Shop - I've seen a lot of people recommend this place.

I second Occansey Designs!!! I just got my custom designed cathedral - here it is:
She is AMAZING. So patient and helpful. She basically helps you design it and once you come up with a final design, she puts it together. She has been incredible. Go to her website and check out all the pics in her portfolio...can't recommend her enough!
