good website for aenvelope etiquette??

I've looked on a bunch of sites (wedding channel, knot, the company i got my invites from, etc...) and while I get basic etiquette on invites, I can't seem to find the answer to my specific questions. Anyone know a site that is really comprehensive with this stuff?? Or the answers to my questions?

1- If someone is a Lieutenant in the US Navy, does their invite get addressed to 'Lieutenant and Mrs. John Smith' or 'Lieutenant, US Navy and Mrs. John Smith?'

2- If someone is in a significant relationship with someone and you are inviting them both, but they don't live together, do you put names on the outer envelope? Or just the person who resides there and put the other person's name only on the inner?

3- Inner envelopes -- what's the deal with these? Should it be 'Mr. and Mrs. Smith' or 'John and Jane Smith' or 'John and Jane'??

TIA!!!!!!
