Health Insurance Question

What if your insurance company requests information, and says in the letter 'If we do not get this information within 45 days we will deny the claim', and you discover this letter (addressed to FH, not you) more than 45 days after the claim was made, and you know DARN well that nothing was done about it?

Should I be prepared to shell out a couple hundred dollars to my doctor's office?
