HELP

Adressing the outside envelopes. Can someone pls show me an example of the wording?
The words along with the 'periods', also the 'and' or should it be '&'.
What is proper/correct???

I did:

Mr. and Mrs. J. Smith
1234 Bell Avenue
West Babylon, New York 11704
